Oxidative stress plays an important role in the pathogenesis of ischemic stroke. Therefore, it is important to develop neuroprotective agents that are able to reduce oxidative stress. Amifostine, as a cytoprotective agent, exhibits neuroprotective effect against ischemic insult by reducing oxidative stress. Several lines of evidence have demonstrated that amifostine neuroprotective effect is closely related to the activation of the transcription factor Nrf2, which induces the expression of antioxidant genes. Here we review the current state of knowledge about the underlying mechanism for the neuroprotective effect of amifostine against oxidative stress and the potential molecular targets that could be involved in these neuroprotective effects. The activation of Nrf2 by amifostine leads to the activation of a variety of cytoprotective genes involved in maintaining cell homeostasis and defense against oxidative stress and, as a result, exerts its neuroprotective effects. Nrf2 activation also leads to the upregulation of extracellular glutamate transporter (GLT-1) which plays an important role in regulating extracellular glutamate level. This review provides an overview of recent findings on the role of Nrf2 in the pathogenesis of cerebral ischemia and therapeutic mechanisms of amifostine and its role in neuroprotection.
